Mission statement:
To assist, coordinate, and achieve optimum levels of fulfillment in providing the necessary support services, maximize third-party benefit enrollment, and improve the overall health and well-being for all individuals.

Services offered:

  • Provides a variety of services from patient-benefits assistance to case management and discharge planning, including advocacy and interpretation for the elderly and disabled individuals
  • Coordinates and manages the Tribal Personal Caregiver Program to offer in-home Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) of personal care to the elderly and disabled persons who seek and want to remain in their homes
  • Assists with and handle durable medical equipment (DME) referrals, such as wheelchairs, walkers, canes, shower chairs, and other related devices in keeping people safe in the community
  • Serves as a liaison in assisting individuals who seek various eligible insurance benefits toward maximizing coverages and minimizing out-of-pocket expenses

Intent:
Uses a strength-based formula that consistently provides daily patient and client support and benefit resource practices that centers around the various care needs for all clients.
